RECCURENT THYROTOXIC HYPOKALEMIC PERIODIC PARALYSIS: LAPORAN KASUS

چکیده

Thyrotoxic Hypokalemic Periodic Paralysis merupakan suatu kondisi langka yang ditandai dengan penurunan kadar kalium darah dan kelumpuhan otot akut akibat hipertiroidisme. Kondisi ini dapat disertai aritmia hingga gagal nafas berpotensi mengancam nyawa, serta memiliki angka kekambuhan tinggi. Dilaporkan laki-laki berusia 54 tahun datang keluhan lemah pada kedua kaki. Riwayat sama terjadi 4 bulan 6 lalu. hipertiroid sejak 20 lalu tidak terkontrol. Teraba nodul di regio tiroid kanan kiri, berbatas tegas, kenyal, ikut bergerak saat menelan nyeri. Indeks Wayne: 9, Free Thyroxine (FT4): 29.87pmol/L, Thyroid Stimulating Hormone (TsHs): <0.005 uIU/ml 2.4 mmol/L. Pemberian terapi drip KCl 50 meq, propranolol 10mg setiap 8 jam, prophytiorurasil 100mg jam. Gejala tirotoksikosis seringkali muncul sehingga menuntut ketajaman klinisi untuk mendiagnosis secara dini. Terapi suplementasi dini memulihkan paralisis mencegah komplikasi lebih berat. Dengan demikian, penting mengetahui manifestasi klinis, patogenesis, tatalaksana, pencegahan fatalnya.
